Burke NC Pre-K Application. Please
note: It is subject to change. Completed
applications will be accepted beginning March 20, 2012.
FY 12-13 Burke NC Pre-K Application
Please fill out the above
application and submit the application, a copy of the birth
certificate, and proof of income to the NC Pre-K site that you would
like for your child to attend.
Children’s Medical
Report
The Children's Medical Report must
be completed by a physician and submitted to the NC Pre-K Office.
This form must be completed within 30 days from the beginning of the
FY 2012-13 school year if it is not submitted with the application.
