County Smart Start’s office located at 304 West Union Street,

note: It is subject to change. Completed
applications will be accepted beginning April 3, 2017.
17-18 Burke NC Pre-K Application
Please fill out the above
application and submit the application, a copy of the birth
certificate, and proof of income to the NC Pre-K site that you would
like for your child to attend.
NC Pre-K Health Form 2017
The Kindergarten Health Assessment must
be completed by a physician and submitted to the NC Pre-K Office.
This form must be completed within 30 days from the beginning of the
FY 2016-2017 school year if it is not submitted with the application.
